Transforming Tradition with a New Commitment

University Hospitals Heather Hill has become University Hospitals Extended Care Campus. Founded in 1939 as Heather Hill Hospital, our beautiful 150 acre campus in Chardon, Ohio, has expanded to a staff of over 500 employees and a bed capacity of 239. The campus was designed to meet the post-acute care needs of northeast Ohio’s elderly, disabled and persons needing rehabilitative and continuing care.

The new name announces three important new developments in the evolution of the Campus’ continuum of care:

  1. University Hospitals’ increasing commitment to serve those who are in need of
    treatment and rehabilitation beyond a traditional hospital stay.
  2. The reinforcement and addition of key values that have made the Campus a premier
    post-acute facility.
  3. Becoming the first free-standing extended care facility in northeast Ohio to offer a Hospitalist Program that provides on-site physician coverage 24 hours a day, every day of the year.

A Comprehensive Brand of Care

Most campuses do not have our continuum of care. That’s an exciting point of differentiation. University Hospitals Extended Care Campus is a destination point, providing a comprehensive array of post-acute care services. In short, it is the most medically complete and complex campus of its kind in the nation.

The Specialty Hospital is our 56 bed long-term acute care hospital and is dedicated the critical care and rehabilitation of the medically complex patients.

Short-Term Rehabilitation (Sub-acute care) offers rehabilitation services for individuals preparing to leave a hospital’s acute care setting, but who require short-term therapy and medical care before returning home.

The Outpatient Rehabilitation and Aquatic Center provides state-of-the-art physical, occupational and speech therapy for patients of all ages. It includes a barrier –free aquatic therapy pool and an underwater treadmill for strength training.

The Corinne Dolan Center for Memory and Aging is nationally recognized as an assisted living facility for residents with early, moderate and severe Alzheimer’s disease or other forms of dementia. The Center’s goals are to maximize an individual’s comfort and enable ongoing successes in a beautiful environment.

The Liberty Assisted Living is an elegant community with 70 spacious, upscale apartments in three connected neighborhoods, resort quality food and access to specialized medical and rehabilitation services.

The Center for Dialysis Care now includes 12 stations that provide dialysis services to patients, residents and the community. The Center’s specialized programs include diet counseling and transplant coordination.


Introducing the new on site Hospitalist Program

We foster a culture of care that is consistent and always improving. The Extended Care Campus now offers 24-hour coverage by highly trained, hospital-based physicians. The Hospitalist Program ensures rapid response and round-the-clock access to a doctor who is familiar with each case and who communicates daily with Campus specialists, nurses and quality management teams, as well as a patient’s family and primary care physician.

The program is the only one of its kind available at any long-term health care facility in northeast Ohio and represents our commitment to optimizing patient safety and positive clinical outcomes.

The Specialty Hospital (LTACH)

The Specialty Hospital at University Hospitals Extended Care Campus (Long-Term Acute Care Hospital, or LTACH) is dedicated to the care of the medically complex patient who requires extended hospitalization and would benefit from an intense rehabilitation program. The Specialty Hospital is JCAHO accredited. We have a high-observation unit and special procedures room in addition to specialized programs to maximize each patient’s recovery.

The Corinne Dolan Center for Memory and Aging

Memory and Aging Services provide several levels of care to accommodate the specific needs of patients and families. This program includes a clinical process whereby UH memory specialists are able to discern, record, and monitor the quality of life indicators on an individual basis enhancing and preserving each resident’s personhood. These levels include:

Rehabilitation Services and Aquatic Center

The University Hospitals Outpatient Rehabilitation and Aquatic Center offers convenient, state-of-the-art physical, occupational, aquatic and speech therapy.

The therapy pool allows barrier-free entry by raising and lowering the pool floor to accommodate wheelchairs or walkers. Features include an underwater treadmill and water jets to create resistance for strength training. Underwater cameras are connected to monitors, computers and videotape equipment to track the patient’s progress.

The Liberty  

The Liberty, an assisted living community that features 70 apartment suites in three connected neighborhood buildings. The Liberty offers luxurious living and common areas, where "front porches" open onto interior neighborhood streets bathed in natural light.
Residents of The Liberty enjoy "catered assistance with living," a special combination of housing, personalized support services, and healthcare designed for individuals who require assistance with activities of daily living.

Susan Juris, President and CEO of the Extended Care Campus, is excited to be directing the transformation that builds on the tradition. “We have heard clearly that patients and families want to be involved in their care.” says Juris. “People come to the campus with medically complex conditions. They’re often here for 30 days or more. Many of them live here year-round, or until they are rehabilitated. In every case, it’s vital that we develop personalized, one-to-one involvement to maximize outcomes and patient/family satisfaction.”

The Christopher House Overnight Accommodations


Christopher House, a picturesque country inn located on the grounds of University Hospitals Extended Care Campus, offers comfortable overnight accommodations for visitors and families.

The attractive guest house opened in 1985 thanks to the generosity of the Christopher Foundation of Chardon, Ohio. The foundation was founded in 1950 by the late Thomas F. Dolan, a pioneer in the packaging industry, who had a keen sense of family as well as a special interest in the needs of the traveler.
