History


University Hospitals Extended Care Campus (formerly known as Heather Hill) was founded in 1939 by a Cleveland social worker, Mabel Woodruff. The original 156-acre estate included 60 acres of timber, five natural springs, several homes, a carriage house, granary and barn. The four wooden buildings on the Extended Care Campus campus are all part of the original estate. Today, these buildings include the Christopher House for overnight visitors, an administrative office building, an education training center and a private residence.

Over the years, the Extended Care Campus has expanded to meet the needs of Greater Cleveland's frail elderly and disabled. Today the Extended Care Campus is one of the largest employers in Geauga County, Ohio, with more than 500 employees. The Extended Care Campus's bed capacity stands at 239.

The Extended Care Campus's multi-level system of services includes the Specialty Hospital (LTACH), Short-Term Rehabilitation, Extended Care, the Corinne Dolan Center for Memory and Aging, The Liberty (Assisted Living), and Outpatient Rehabilitation and Aquatic Center. The facility is non-profit and non-sectarian and operated by a board of trustees.

The Extended Care Campus is recognized throughout the world for its state-of-the-art Alzheimer services. The Corinne Dolan Center for Memory and Aging serves persons in the early and middle stages of this devastating illness through respite care, short-term overnight care and residential services. It was designed by architect Stephen Nemtin of the Frank Lloyd Wright Foundation to maximize independence and promote a high quality of life. It has drawn more than 8,000 visitors from around the world since its opening in 1989.

A Special Care Center for persons in the later stages of Alzheimer's disease and other dementias opened on the Extended Care Campus campus in 1993. It provides around-the-clock medical care and specialized programming that builds upon each individual's unique abilities.

The Extended Care Campus's rehabilitation programming serves adults and adolescents recovering from head injury, stroke, spinal cord injury, amputation, orthopedic surgery, neuromuscular disease and other major illnesses.

The Specialty Hospital serves the medically complex patient who would otherwise remain in the acute care setting. Short-term rehabilitation care provides outcome oriented medical and rehabilitative care for persons who are too frail to return home following acute hospitalization but whose condition no longer warrants the resources of an acute hospital. Programs may be continued or initiated on an outpatient basis.

The Extended Care Campus serves as a teaching facility and recently opened an on-campus residence for student interns. The facility maintains working relationships with teaching hospitals and universities in the United States and Great Britain.

DIRECTIONS FROM CLEVELAND AREA

Cleveland Airport
Take Route 480 East to I-271 North. Exit on Mayfield Road East (Route 322). Proceed east on Mayfield through Chesterland (passing Route 306) until you reach Bass Lake Road. Turn left on Bass Lake Road. The Extended Care Campus' entry lane is approximately 1/4 mile down Bass Lake Road on the left.

West Side
Take I-71 North to I-90 East. From I-90 East follow I-271 South. Exit on Mayfield Road East (Route 322). Follow Mayfield Road past Alpine Valley Ski Lodge to Bass Lake Road. Turn left onto Bass Lake Road and follow to the Extended Care Campus Entrance.

South Side
Take I-77 North to I-90 East. Follow West Side directions from Mayfield Road East exit (Route 322).

Willoughby, Mentor Painesville
Take Route 2 West to 1-271 South and follow West Side directions from Mayfield Road East exit (Route 322) or take 1-90 or Route 2 East to 44 South around Chardon Square to Bass Lake Road (past Chardon Lakes Inn). Turn right.

East Cleveland
Euclid Take Route 2 East to I-271 South and follow East Side directions from Mayfield Road East exit (Route 322).

Ashtabula, Geneva, Madison
Take I-90 West to 44 South around Chardon Square to Bass Lake Road (past Chardon Lakes Inn). Turn right.

Akron, Bedford, Beachwood
Take I-77 North from Akron to I-271 North. Follow I-271 North to Mayfield Road East (Route 322) and follow West Side directions from Mayfield Road East exit.

Parma, Garfield Hts., Warrensville Hts.
Take 480 East to I-271 North and follow West Side directions from Mayfield Road East exit (Route 322).

Streetsboro, Ravenna, Solon
Take 480 West to I-271 North and follow West Side directions from Mayfield Road East exit (Route 322).

DIRECTIONS FROM WARREN AREA
Take 422 West to 44 North. Follow 44 North to Route 322 West. Follow Mayfield Road to Bass Lake Road and turn right toward entrance.

DIRECTIONS FROM CINCINNATI AND COLUMBUS AREAS
Take I-71 North to I-271 North to Mayfield Road East exit (Route 322). Follow Mayfield Road East past Alpine Valley Ski Lodge to Bass Lake Road. Turn left onto Bass Lake Road and follow to the Extended Care Campus entrance.

DIRECTIONS FROM WESTERN PENNSYLVANIA
Erie
Take I-90 West to 44 South around Chardon Square to Bass Lake Road (past Chardon Lakes Inn). Turn right on Bass Lake. Proceed to the Extended Care Campus entrance.

Western PA
Take Pennsylvania Turnpike (76) to Ohio Turnpike (80). Follow Ohio Turnpike to Exit 13-A. Proceed north on Rte. 44 to Mayfield Rd. (Rte. 322). Turn left on Mayfield. Proceed approximately 2 miles to Bass Lake Road. Turn right on Bass Lake. The Extended Care Campus entrance lane is on the left.

 

Visiting Hours

Family and friends are encouraged to visit University Hospitals Extended Care Campus patients and residents to reinforce our focus on harmony and healing. Visiting hours vary by area and are designed to avoid conflicting with the therapy process. Alternative plans may be necessary if a resident is ill. General guidelines include the following:

Specialty Hospital (Areas D & E) - from 4 to 8 p.m. daily and 12 to 8 p.m. on weekends

Short-Term Rehabilitation (Areas A & B) - 8:00 a.m. to 8:00 p.m., but please call ahead for individual patient therapy schedules

Extended Care (Areas C and F), Special Care Center and the Corinne Dolan Center for Memory and Aging - 8:00 a.m. to 8:00 p.m.
